What is social innovation net
The project involves the development of an interactive digital platform aimed at networking the six municipalities of Ditretto 22 and supporting their operations under PON Inclusion. The processes of: social secretariat, assessment and multidisciplinary team (é.m.), case management, drafting of individualized and family care plans, orientation and accompaniment, monitoring, to the coordination of all the actors of the network of services activated for the integrated taking charge are engineered. The platform digitizes and integrates municipal social services, ensuring not only interoperability between institutional BDs but also application cooperation, multi-agency and multi-level linkage, all in compliance with the latest legislation on privacy and protection of sensitive data.
